AzCHOW empowers Arizona's Community Health Workers through advocacy and specialized training—bridging the gap between underserved communities and the healthcare system to improve health equity for all.

We work together to promote, protect, and improve the health and quality of life through exceptional public health services and education. We will promote and support the health and well-being of the public both for now and for future generations.

SEAHEC's vision is to serve as a hub for health advocacy, education and action in rural and underserved communities in Southeast Arizona. Through our initiatives, partnerships and programs, we reach out to and collaborate with communities, families and individuals. An effective way to do this is through the utilization and support of community health workers (CHWs). Our most recent program initiatives have CHWs at the heart.

Helping to maintain the good health and happy lives of Santa Cruz County residents is the primary purpose of the Mariposa Community Health Center. Since our founding in 1980 we have become the largest and most comprehensive provider of medical, dental, disease prevention and health promotion services in SE Arizona. Our locations in Nogales, Rio Rico, Tubac and Patagonia offer a “one stop wellness” model of integrated care that recognizes and responds to the whole person from physical to social to psychological needs. Our services include adult/internal medicine, pediatrics, obstetrics and gynecology, dentistry, behavioral health and two full-service pharmacies. Our community outreach stems from our Community Health Services Department (formerly Platicamos Salud), which includes an entire building devoted to WIC for Women, Infants and Children. For a family of one or a family of many, Mariposa is here for you.

Circles of Peace is a community-based 501c3 organization dedicated to healing, connection, and transformation. Through restorative justice, prevention, and community programs, we work alongside youth, families, and community partners to build safer, healthier, and more compassionate communities across Santa Cruz County. Grounded in trauma-informed and community responsive care, we believe that lasting change begins with understanding, dignity, and the power of community. Together, we are building peace—one circle at a time.

Verde Valley Caregivers Coalition commits to supporting and encouraging older adults in their quest to age in place in their community of choice by enhancing their independence, promoting higher quality of life, and reducing barriers to happiness, making the Verde Valley an attractive and sustainable retirement destination.

Chiricahua Community Health Centers, Inc. is a mission-driven Federally Qualified Health Center and a tax-exempt not-for-profit 501(c)(3) organization. Founded in 1996 as a small, rural health clinic operating in a community center, Chiricahua has since grown to become the largest primary care organization in southeastern Arizona, serving more than 31,000 patients annually. Chiricahua operates fifteen fixed-site medical, dental, behavioral health, and pharmacy clinics, and eight mobile-medical and mobile-dental units, that serve patients throughout the more than 6200 square mile borderlands of Cochise County.

The Graham County Health Services purpose is to promote health and quality of life for all Graham County residents through education, service and leadership.

Desert Senita Community Health Center, named after the resilient Senita cactus, is a nonprofit Federally Qualified Health Center dedicated to serving rural Arizona Southwest communities regardless of insurance status. As the Cactus Family, we provide affordable, high-quality care, walking alongside patients through every step of their health journey to build a stronger, healthier community Desert Senita's roots starts at the Ajo Community Health Center dba Desert Senita Community Health Center which evolved from a hospital and outpatient facility which was run by the New Cornelia Copper Company which as a large mining company that started in 1854 until it ceased local operations in 1986.

The mission of Canyonlands Healthcare is to promote healthier lives through affordable, accessible and integrated primary care. Canyonlands Healthcare is a private, not-for-profit corporation providing quality primary and preventive care. We provide comprehensive and cost-effective healcare services to 10 locations in 8 communities in Arizona and Nevada.

The mission of the Winchester Heights Health Organization is to improve the well being of the residents of Winchester Heights, specifically through the following activities: * Promoting and delivering educational workshops on maintaining or improving physical, mental and economic well-being; * Creating recreational programs; * Serving as a the community resource for technology and educational materials; and * Maintaining and operating its principle offices as the center for community social services.

RAzCHOW Member Organizations (Non-rural)

Campesinos Sin Fronteras’ mission is to promote the overall well-being of agricultural families and low—to moderate-income individuals by providing and facilitating access to health, behavioral health, social services, housing rehabilitation, education, and workforce development.

Native Americans for Community Action, Inc. (NACA) is a 501(c)(3) non-profit Urban Indian Organization, (UIO). NACA is in Flagstaff, Arizona. The organization was founded in 1971, to create a place for American Indians and Alaskan Natives (AI/AN) to receive support away from their homelands. NACA serves as the only UIO in the Indian Health Services (IHS) Navajo area, which is the largest IHS provider area in the nation, servicing 244,000 people. Currently, NACA is not only seen as a hub for healthcare and support but also a way for those away from their homelands to establish connections and belonging all while being immersed in cultural traditional education and practices. Through the implementation of cultural education and practices braided with contemporary approaches, we can address the wholistic wellbeing of the AI/AN community. Ultimately, striving to reduce the Native American health disparities from chronic health conditions. The mission of Native Americans for Community Action, Inc. is to provide preventive wellness strategies, empower, and advocate for Native people and others in need to create a healthy community based on Harmony, Respect, and Indigenous values.

The Mission of the Arizona Advisory Council on Indian Health Care (AACIHC) is to serve as a resource for all Tribal governments and the State of Arizona by supporting prevention, training, education, workforce development, policy and legislation to meet the unique health care needs of American Indian and Alaska Native (AI/AN) population in Arizona. We seek to educate and advocate for improved health outcomes. The AACIHC strives to be recognized as a trusted resource on health equity for all Tribal Nations and American Indian communities throughout Arizona.

The state’s lead agency for promoting and protecting the health of all Arizonans. We set the standard for personal and community wellness through disease prevention, emergency preparedness, and the licensing of healthcare and childcare facilities—ensuring a safe, healthy, and resilient Arizona for every resident.

RAzCHOW Advisory Organizations

Committed to finding solutions to end health disparities regionally, nationally, and globally by engaging researchers, healthcare providers, and communities in new ways that are culturally relevant and respectful. Our collaborative work provides solid information that our partners can use for their work to improve health and well-being for all.

Arizona Complete Health builds upon a tradition of excellence started by Health Net of Arizona and Cenpatico Integrated Care. Health Net of Arizona was founded in Tucson in 1981 as Intergroup of Arizona, and has a proud history of serving Arizonans statewide through Medicare Advantage, employer-based coverage, Marketplace and AHCCCS. Cenpatico Integrated Care began serving Arizonans in 2005, with a special focus on improving access to community-based behavioral health services and emphasizing person-centered, whole health care for individuals. Arizona Complete Health has a commitment to improving the health of the community one individual at a time through affordable and reliable health care plans. We do this through our focus on the individual, whole health and local involvement.

The Arizona Prevention Research Center (AzPRC) at the University of Arizona has been a continuously funded PRC for 25 years, spanning five funding cycles. Through strong partnerships with community organizations, AzPRC conducts research and demonstration projects that integrate the Community Health Worker (CHW) workforce to improve health outcomes in underserved populations across the U.S.-Mexico border region and beyond. AzPRC's projects focus on: Chronic disease prevention and management Community-driven health promotion Health equity and social determinants of health Binational and border health initiatives Indigenous health and resilience By leveraging community engagement and participatory research methods, AzPRC continues to develop evidence-based interventions that address pressing public health challenges while strengthening the capacity of local communities to lead sustainable health initiatives.
